Position Summary

As an LPN at Charter Senior Living, you are a vital part of our care team and a trusted advocate for our residents. You will support residents in Assisted Living and/or Memory Care by delivering compassionate, person-centered nursing care that honors each individual’s life story, dignity, and independence.

Key Responsibilities

Perform resident assessments and assist with care planning
Administer medications and treatments accurately and safely
Document nursing services and resident responses
Support and evaluate individualized plans of care
Collaborate with team members to ensure exceptional resident experiences

Qualifications

Active LPN license in the state of Illinois
