Department Overview

Cascades East is located in Klamath Falls, Oregon, providing an incredible rural medicine experience. Residents enjoy numerous outdoor opportunities and the scenery of the Cascade mountain range. The clinic is a certified Patient Centered Primary Care Medical Home, with an emphasis on team-based care and innovation. Sky Lakes Medical Center, the hospital at which the Cascades East Family Medicine Center is based, is the only hospital for the Klamath Basin, which serves over 100,000 people. The combination of a strong medical community and populations with significant health care needs makes Klamath Falls a unique setting to practice primary care. Providers work within a collaborative interprofessional team to practice the full scope of family medicine, often caring for entire families from newborns all the way to grandparents.

The appointee shall provide services as assigned by the supervisor in furtherance of the university's missions and goals of teaching, research, patient care, outreach and public service.

Compensation for the position will be commensurate with experience and other rural practice sites.

Function/Duties of Position

The provider will be involved in eight half days with direct patient care, and the supervision of students and other university learners at the Cascades East Family Medicine Center. A half of a work day is provided for administration and a half of a work day is provided for scholarly activities. The expected work week for this position will be flexible with rotating Saturdays and some of the weekday hours would be in the evening.

The faculty member will be expected to have a strong commitment to developing the practice into a vibrant clinical and academic operation, which is successful from both a professional and business point of view.

The faculty member will be expected to participate in continuing education, professional and faculty development and scholarship.

The Duties Include

  • Work collaboratively with full and part time faculty to maintain an active practice
  • The Appointee will directly see patients in their clinic eight half days per week and have two half day’s available for administrative duties.
  • Expectations for number of patients seen per half day will be commensurate with that of other faculty and with minimum target numbers of 8-10 visits per half day depending on complexity of the patient population.
  • The Appointee will be expected to document using the electronic health record and maintain complete and timely patient care documentation consistent with OHSU and Cascade East Family Medicine Center and Sky Lakes Medical Center policies and procedures.
  • Be available for teaching activities.

Required Qualifications

  • Current Nurse Practitioner license for the state of Oregon, State of Oregon Nurse Practitioner Prescriptive Privileges, and DEA certification.
  • Current Physician Assistant license for the state of Oregon, Prescriptive privileges, and DEA certification.

Department Overview The Division of Gastrointestinal and General Surgery is a comprehensive team of surgeons focused on the areas of colorectal surgery, esophageal surgery, hernia surgery, treatment of pancreatic cancer and surgical nutrition. Our general surgeons support numerous clinical research trials and actively partner with scientists in the community to develop innovative surgical devices. The general surgery division also encompasses one of the largest residency programs in the country, as well as fellowships in minimally invasive surgery and colorectal surgery. The Advanced Practice Provider (APP) in this role will deliver consistent, high-quality medical management to inpatients on the Abdominal Wall Surgery service, primarily within acute care units. The APP will work collaboratively with attending faculty, residents, interns and the broader interdisciplinary care team to provide comprehensive, patient-centered care, with a focus on clinical excellence, care coordination, and efficient discharge planning. Function/Duties of Position Clinical Duties- Assure the safe, efficient, high-quality quality and patient-centered management of care to patients, this includes but is not limited to: Completion of comprehensive assessment of the patient’s status; developing, monitoring, and revising, as appropriate, an evidence-based treatment plan that addresses diagnoses, patterns of disease, risk factors, and treatment alternatives; Interpret diagnostic tests, prescribe and manage medications and other therapies, provide health teaching with an emphasis on prevention of illness and health maintenance; and, refer patients to other healthcare professionals as necessary. Collaborate with interdisciplinary healthcare teams to diagnose and treat abdominal wall surgery patients primarily in an inpatient environment. Advanced Practice Provider

Department Overview The Division of Gastrointestinal and General Surgery is a comprehensive team of surgeons focused on the areas of colorectal surgery, esophageal surgery, hernia surgery, treatment of pancreatic cancer and surgical nutrition. Our general surgeons support numerous clinical research trials and actively partner with scientists in the community to develop innovative surgical devices. The general surgery division also encompasses one of the largest residency programs in the country, as well as fellowships in minimally invasive surgery and colorectal surgery. The Advanced Practice Provider (APP) is a faculty member credentialed and privileged as a Physician Assistant (PA) or an Advanced Practice Registered Nurse (APRN). The APP is a member of the OHSU Professional Staff practicing in accordance with the respective bylaws. The APP shall provide services as assigned by the supervisor in furtherance of the university’s missions and goals of teaching, research, patient care, outreach and public service. The APP is expected to comply with scope of practice, other laws, regulations, OHSU policy, and other applicable professional standards of practice. Function/Duties of Position Clinical Responsibilities Inpatient Responsibilities: Providing inpatient care and follow-up of assigned surgical patients from the operating room (OR) to admission, and through discharge. Includes, but is not limited to: rounding on a daily basis with the faculty, developing and implementing, in conjunction with faculty physicians, clinical plans of care and/or protocols for inpatients on the General Surgery-Hernia/Abdominal Wall service with common conditions/diagnoses/procedures; educating the house staff and nursing unit staff on these protocols, with the goal of enhancing quality of care, enhancing quality of patient services, and decreasing length of inpatient stay; participating with hospital/unit staff in evaluating results of these protocols and continually improving effectiveness of same; working actively with hospital and unit staff to rapidly develop and implement discharge plans for inpatients on the General Surgery-Hernia/Abdominal Wall service and to address length of stay issues; promoting and maintaining health of inpatients on the General Surgery--Hernia/Abdominal Wall service through the use of ancillary services, educational materials, and counseling of patients and families; providing resident and intern training/education in utilization of plans of care to improve patient services and outcomes of treatment; providing and/or participating in clinical in-service training and staff educational programs for the inpatient service. Procedural duties include, but are not limited to: wound care (dressing changes, debridement, washout), stoma-related services, IV & other line placement, catheter or enteral tube placement and/or exchange, drain management, other. Appropriate billing for various inpatient duties, as outlined above. Following all patient care standards, as outlined above. Operating Room Responsibilities -Assistant Duties For APP designated with OR responsibilities: Provide operating room and procedural assistance to General Surgery-Hernia/Abdominal Wall surgeons as determined by the Division Head or their designee(s). Includes assistance in performing a variety of general surgery procedures, with a focus on hernia repair and abdominal wall reconstructive procedures, among other surgical procedures, various suturing, wound closure, dressing application, postoperative monitoring, postoperative orders and discharge. Appropriate billing for various operating room duties, as outlined above. Outpatient Providing patient care in the outpatient setting for preoperative and postoperative care, wound care, unscheduled visits, and chronic follow-up as determined by the faculty. This includes but is not limited to: developing patient care plans with the faculty and house staff to ensure continuity of care and enhance quality of outpatient care. Training and supervision of General Surgery-Hernia/Abdominal Wall clinic team staff, house staff and medical students. Promoting and maintaining health using ancillary services, educational materials, and counseling of patients and families as outpatients; providing same day response to medical advice/prescription phone requests and coordinating patient services with outpatient patient care staff and other departmental advanced care providers. Promoting a safe environment and adhering to policies and confidentiality to support the mission and goals of OHSU. Recognizing clinical emergencies or urgent situations and acting autonomously, efficiently, and effectively to provide situational resuscitation and/or treatment. Assessing patients for outpatient clinic visits, determining medical necessity of visit and ensuring a course of action based on department and OHSU policies and/or in collaboration with the faculty. Utilizing resources and educational materials for patients and families; consulting with the faculty to provide up-to-date condition-specific health care information for outpatients. This includes prompt and professional communication, face-to-face customer contact skills, crisis management, use of available information technology, standard complaint processing, flexibility in coverage of internal service needs, and continuous application of process improvement methods and skills. Procedural duties include but are not limited to: wound care (dressing changes, debridement, washout), stoma-related services, drain management & removal, Foley catheter or enteral tube placement and/or exchange, other. Appropriate billing for various outpatient duties, as outlined above. Required Qualifications Master’s Degree as required by specific Advanced Practice Provider role. Minimum two years clinical experience, including wound care experience. Current BLS Certification (ACLS a plus) Registration with the Oregon Board of Medical Examiners as a licensed Physician Assistant, or a Nurse Practitioner license from the Oregon Board of Nursing; Drug Enforcement Association certification required. Regular and consistent attendance at work Additional Details Due to the nature of service, hours will vary with occasional weekends required. Occasional 24-hour call availability may be required. Frequent interruptions in an environment of frequent change and fluctuations. May be exposed to excessive auditory and visual stimulation. May be required to attend out of town conferences according to OHSU travel and reimbursement policies. May be exposed to body fluids, tissue, possible radiation exposure, and infectious processes. Subject to computer work for several hours, as needed. Able to use all appropriate patient care equipment and office equipment within work area.
