Encore Village of Schaumburg is currently hiring for Licensed Practical Nurses (LPN) in our Reflections unit. With set schedule of 12 hour shifts. Both days and night shifts. The primary role of the LPN is to perform day-to-day nursing activities in the provision of direct resident care. LPN's perform these duties within the scope of practice that they are licensed to perform. All nursing activities must be performed in accordance with federal, state, and local guidelines and regulations that govern our facility, including the policies and procedures of the organization.

Responsibilities include:

Performs resident assessment through use of direct physical assessment, behavior monitoring, various assessment forms, review of hospital records, laboratory results, resident medical records, and information provided by the interdisciplinary team, resident and/or the resident's significant others.
Monitors residents for changes in condition that may indicate progress or deterioration in the resident's overall status. Notifies physicians and families of changes and transcribes orders received.
Administers medications according to facility policy and monitors residents for therapeutic and adverse effects. Notifies physician and pharmacy of suspected and actual adverse reactions.
Triages emergency situations and provides appropriate first aid in a timely manner. Completes appropriate reports and consults the ADON or charge nurse on duty.

Qualifications/Requirements:

Current LPN License within the State of Illinois
Graduation from an accredited LPN program
Must have a minimum of 2 years experience in long term care or a skilled nursing facility
Current CPR Card

Assist in checking medication regardless of packaging system. Assess the residents to determine need for “as needed medication” and appropriately document and report to supervisor. Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse.
