Home RN Home Care Case Manager (Full-Time, Day Shift)

Location: Springfield, OR.

FTE: 1.0 | Schedule: Day

Hourly Range: $45.42 – $71.98

Make a meaningful difference—one home visit at a time. PeaceHealth is seeking a Registered Nurse – Home Health Visiting Nurse to join our compassionate care team in a part-time (0.80 FTE), day-shift position.

As a Home Health RN, you’ll bring skilled, personalized care to patients in the comfort of their own homes. This role requires critical thinking, independent judgment, and compassionate communication as you evaluate and care for individuals with diverse needs across all stages of recovery and illness. You’ll collaborate with an interdisciplinary team to support patient goals, improve quality of life, and promote healing at home.

Key Responsibilities:

  • Conduct home visits to assess patients and determine eligibility for care
  • Develop and manage individualized care plans in collaboration with the care team
  • Deliver direct nursing care and patient/family education per physician orders
  • Monitor progress, adjust plans as needed, and coordinate services across disciplines
  • Document timely and accurate clinical information per regulatory guidelines

Qualifications:

  • Graduate of an accredited nursing program (BSN preferred)
  • Minimum 2 years acute care or hospice/home health experience required
  • Current RN license in state of Oregon
  • Valid driver’s license, proof of auto insurance, and BLS certification required
  • Strong assessment, organizational, and communication skills
