Position Summary: Assessment Nurse (Part-Time RN)

The Assessment Nurse (RN) plays a vital role in ensuring residents receive the best quality of care possible. You’ll conduct comprehensive health and wellness assessments, determine levels of care, and collaborate with interdisciplinary teams to promote resident well-being — all while maintaining compliance with Connecticut state regulations.

You’ll work closely with our Health and Wellness Director and Executive Director to ensure residents and families experience the highest level of support and confidence in their care.

Key Responsibilities

  • Perform initial and ongoing resident health assessments

  • Develop and update individualized care plans based on assessment outcomes

  • Communicate effectively with residents, families, and healthcare providers

  • Support compliance with state regulations and company policies

  • Collaborate with clinical and operations teams to ensure quality and continuity of care

  • Participate in staff training, audits, and wellness initiatives

  • Step in to support resident care or administrative needs as required

Qualifications

Licensure: Current, active Registered Nurse (RN) license in the State of Connecticut (required)
Experience: Minimum 1–2 years of nursing experience in assisted living, memory care, or long-term care settings
Skills: Strong assessment and documentation abilities; excellent interpersonal and organizational skills
