Are you a Nurse Practitioner with a passion for rehab medicine, post-acute care, and functional recovery?

As a Physiatry Nurse Practitioner, you will partner closely with a supervising Physiatrist and the interdisciplinary care team to manage the medical and rehabilitative needs of patients recovering from surgeries, neurological events, or other function-limiting conditions. You’ll play a key role in improving outcomes by facilitating smooth recoveries, reducing re-hospitalizations, and promoting independence.

Job Requirements:

  • Active and unrestricted Nurse Practitioner license in the state of Illinois

  • Board-certified as an Adult, Geriatric, or Family Nurse Practitioner

  • Master’s Degree in Nursing from an accredited program

  • Experience in PM&R, geriatrics, internal medicine, or SNF settings preferred

  • Strong clinical skills in functional assessments, pain management, and rehab-focused medical care

  • Comfortable working both independently and in collaboration with a supervising physician

  • EMR proficiency and commitment to timely charting and care coordination

Key Responsibilities:

As the Physiatry Nurse Practitioner, you will serve as a functional medicine specialist in the facility:

  • Performing comprehensive assessments focused on rehab goals, mobility, and pain

  • Developing and updating individualized treatment plans alongside the supervising Physiatrist

  • Conducting regular follow-ups to monitor progress and modify plans of care

  • Participating in interdisciplinary care team meetings and patient rounds

  • Collaborating with therapy teams to track rehabilitation milestones and address barriers to progress

  • Educating patients and families about diagnoses, recovery goals, and functional strategies

  • Documenting visits, interventions, and outcomes clearly and promptly in the EMR

  • Supporting discharge planning and care transitions to ensure continuity of rehabilitation progress

  • Serving as a clinical resource to staff for musculoskeletal and neurologic conditions

Salary & Compensation:

Annual Pay: $105,000 - $130,000 based on experience
