Position Overview: We are seeking a compassionate and detail-oriented Registered Nurse Assessment Coordinator to join our team. The RNAC will play a critical role in assessing and documenting patient needs, coordinating care plans, and ensuring regulatory compliance. Key Responsibilities:

  • Conduct comprehensive patient assessments to determine care needs.
  • Collaborate with healthcare professionals to develop and update individualized care plans.
  • Ensure compliance with Medicare and Medicaid regulations and guidelines.
  • Accurately complete and submit MDS (Minimum Data Set) assessments.
  • Monitor patient progress and adjust care plans as necessary.
  • Provide education and support to clinical staff regarding assessment and documentation best practices.
  • Participate in interdisciplinary team meetings to discuss patient care and coordination.
  • Maintain accurate and up-to-date patient records.

Qualifications:

  • Current Registered Nurse (RN) license in Arkansas
  • Minimum of one year of experience in long-term care or similar healthcare settings.
  • Strong knowledge of MDS assessments and Medicare/Medicaid regulations.
  • Excellent communication and interpersonal skills.
  • Detail-oriented with a commitment to accuracy and compliance.
  • Ability to work collaboratively in a team-oriented environment.
  • Proficient computer skills and familiarity with electronic health records (EHR) systems.

MDS Coordinator, RN

As a MDS Coordinator you will conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility. Responsibilities and Duties Include: Conduct and coordinate the development and completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs. Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the RAI process. Develop, implement, and maintain an ongoing quality assurance program for the resident assessment. Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur. Ensure that a current copy of the RAI Manual is available to persons completing portions of the MDS. Ensure that all assessments are completed and transmitted in a timely manner.
