Job Description:
Under the direction of the Institution's CEO/CNE or designee, Temporary/Relief LVN shall provide services as permitted within the scope of practice for Licensed Vocational Nurse services in accordance with Institution/Facility policies and procedures. Duties/Responsibilities shall include, but are not limited to:

  1. Organize, provide, and maintain necessary healthcare services consistent with CCHCS policies and procedures;
  2. Conduct basic hygiene and nursing care, including administration of prescribed medications and the therapeutic effects, treatments, and disease prevention, as ordered by the physician;
  3. Collect information regarding patient status and report the results to the Primary Care Provider (PCP) or RN in a timely manner;
  4. Provide patient/youth education and accurately document care in the approved format, noting and carrying out physician's orders;
  5. Administer/assist with therapeutic measures and treatments;
  6. Clean and maintain equipment;
  7. Obtain specimens for diagnostic testing as ordered;
  8. Assist the RN and PCP with clinic appointments; e.g., physical exams, minor surgeries, casting, identifying minor illnesses;
  9. Assist mental health professionals by contributing to the treatment planning process and the formal evaluation of patient/youth progress by monitoring and recording observed patient behavior;
  10. Participate in the training of patient/youth in activities of daily living and assist in routine treatments; and
  11. Document in the patient/youth health record the nursing care and treatment provided and the patient/youth response to facilitate the continuity of care and communicate to other healthcare providers.

Establishes contact with the assigned ECM providers and/or community provider to discuss each patient’s re-entry plan including their health needs, risks, and re-entry requirements. g. Shares detailed information from the Health Risk Assessment with the ECM provider, ensures that all critical health issues, potential risks, and specific re-entry needs are communicated clearly to facilitate comprehensive care coordination and re-entry care planning. h. Obtains the re-entry care plans and resources developed or identified by the ECM provider to facilitate the process of connecting patients to necessary services and support systems in the community to assist with successful reintegration into the community. i. Collects data to assist with finalization of the of the re-entry care plans into the Electronic Health Record System (EHRS). j. Facilitates warm handoffs between healthcare staff and community providers to ensure a smooth and supportive transition for patients. Supports the establishment and implementation of telehealth services and scheduling to ensure successful warm handoff with the patient and community providers. l. Completes Professional to Professional clinical handoffs with community providers and the patient in the county of release for patients with substance use disorder (SUD) as appropriate. m. Uses effective communication to provide patient education and addresses any questions or concerns regarding their re-entry care needs, managing chronic conditions, medication adherence, preventive health measures, and navigating community resources to support re- entry needs. n. Escalates clinical abnormalities or concerns to the appropriate primary care team member and community provider. o. Utilizes decision support tools such as master registry, patient summaries and scheduling and diagnostics reports to monitor effectiveness of treatment planning and adjusts course of care in collaboration with the care team and community provider. Serves as a patient advocate. p. Engages in ongoing training and professional development to stay current with best practices in correctional healthcare and the CallAIM Justice Involved Initiative. q. Builds relationships and enhances communication between community partners and providers. r. Engages in critical thinking and creative problem solving, continuously aligns program goals with organizational goals. s. Participates in quality improvement activities such as chart reviews, audits, and participation on the Quality Improvement Team. t. Assists in the development, revision, and implementation of all policies and procedures as directed by the supervisor or designee. u. Appropriately elevates concerns to supervisor to hold stakeholders accountable. v. Maintains a safe and secure work environment and follows all safety precautions and Department policies and procedures; reports any unsafe equipment or inappropriate conduct and/or activity to management. w.
