As a Licensed Practical Nurse, your main role will be to deliver nursing care to our residents within a warm, comfortable, and home-like environment.





Responsibilities





  • Accurately assesses, communicates, and documents residents’ status.


  • Observes and reports any significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations.


  • Directs and supervises the daily work assignments of the Resident Care Assistants.


  • Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job.


  • Coordinates care needs with community providers via an effective case management process.


  • Effectively balances service demands with supporting resident independence.


  • Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations.


  • Responds to emergencies and personal emergency response system and knows when to call for backup.




Requirements





  • Current state license as an LPN and CPR certification


  • Graduate of an approved LPN program (per state requirements)


  • Minimum of 1 to 2 years of experience working as a nurse


  • Previous experience working with the elderly in a paid or volunteer position is highly desirable

Job description Overnight Weekend Supervisor LPN/RN Part time Position at Atlas Healthcare at Daughters of Miriam Purpose of Your Job Position: The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs and other nursing personnel. To monitor the performance of CNAs, nursing and non-licensed personnel, provide education and counseling, perform disciplinary action as necessary, and complete performance evaluations. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our Center, and as may be required by the Director of Nursing, Assistant Director of Nursing, or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times. Overnight Weekend Supervisor LPN/RN Duties and Responsibilities: Administrative Functions Direct the day-to-day functions of the CNAs or other nursing personnel in accordance with current rules, regulations, and guidelines that govern the long-term care Center. Ensure that all assigned nursing personnel including CNAs comply with the written policies and procedures established by this Center. Periodically review the department’s policies, procedure manuals, job descriptions, etc. Make recommendations for revisions. Provide discipline and evaluations of assigned CNAs and other nursing personnel in accordance with current rules, regulations, and guidelines that govern the Center. Issue verbal and written disciplinary warnings to assigned CNAs and other nursing personnel for violations of current rules, regulations, and guidelines of the Center. Infection Preventionist/Wound Nurse (RN/LPN)

Major Duties and Responsibilities Develops and implements an ongoing infection prevention and control program to prevent, recognize, and control the onset and spread of infections in order to provide a safe, sanitary, and comfortable environment. Establishes facility-wide systems for the prevention, identification, reporting, investigation, and control of infections and communicable diseases of residents, staff, and visitors. Develops and implements written policies and procedures in accordance with current standards of practice and recognized guidelines for infection prevention and control. Oversees the facility’s antibiotic stewardship program. Oversees resident care activities that increase risk of infection (i.e., use and care of urinary catheters, wound care, incontinence care, skin care, point-of-care blood testing, and medication injections). Leads the facility’s Infection and Prevention Control Committee. Develops action plans to address opportunities for improvement. Participates on the facility’s QAA Committee. Performs duties as assigned. Implements an annual infection control risk assessment process and participates in the facility’s annual facility assessment review process. Reviews and/or revises the facility’s infection prevention and control program, its standards, policies, and procedures annually and as needed for changes to the facility assessment to ensure they are effective and in accordance with current standards of practice for preventing and controlling infections.
