Your Role as a Homecare Nurse:

As a Homecare Nurse (RN or LPN/LVN), you will provide skilled nursing care to patients in their homes. Your responsibilities will include:

Developing and following individualized care plans
Administering medications and performing wound care
Monitoring patients’ conditions and updating care teams
Educating patients and families on managing conditions
Offering emotional support and ensuring comfort
Experience and comfort with Tracheostomy Care

Our goal is to provide compassionate care that enhances the overall health and well-being of our patients.

Schedule: Full Time/Weekday/Dayshift

Qualifications:

Associate’s Degree in Nursing
Current BLS certification
At least one year of nursing experience (home care experience preferred)
Experience with G-Tube care

Participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the department and within the parameters of the licensing board. Communicates and works collaboratively with members of the multidisciplinary team i.e., medical staff, administration, clinicians, dietitians, and milieu personnel. Takes reasonable care and precautions to ensure high client safety standards and a therapeutic environment exist. This position is onsite at the facility and is not a remote position. Roles and Responsibilities: ESSENTIAL FUNCTIONS: Provides direct and individualized nursing care to an assigned group of clients under the supervision of a registered professional nurse, while always maintaining the assurance of client safety and following unit and facility policies and procedures. Complies with all company safety policies and practices. Assists in relevant health and psychiatric teaching for client and family. Serves as a role model for clients and peers.
