Job Description Summary

The TravelingRegistered Nurse plans, organizesand directs home care services and is experienced in nursing, with emphasison community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Traveling Registered Nurse Assignment Details:

  • Assignment Duration:Standardtravel assignments are 13 weeksin duration unless otherwise agreed upon in advance.
  • Weekly Hours:Full-time schedule of 40 hoursper week.
  • Starting Location:[To be determined– specify region, city, or service area if known.]
  • Work Schedule:Schedule will vary based on assignment location. Weekend availability may be required depending on patientneeds.
  • Employment Status:Thisposition is directofBridge Home Health & Wellness. Upon completion of an assignment, the nurse may be reassigned to a new contract within the State of California as determined by organizational needs.
  • Supervision Responsibilities:Instructs, supervises, and evaluates home health aide care every two (2) weeks to ensure compliance with the plan of care and quality standardsand LVN every sixty (60) days.
  • Attendance & Reliability:Must adhere to company attendance and reliability policies, which are essential for delivering high-quality and consistent patient care.
  • Training & Team Support: Demonstratesa willingness to assistin onboarding and training new employees as requested and serves as a thought leader by sharing knowledge and best practices with the team.

Essential Functions/ Responsibilities:

Patient Care:

  • Completes an initialassessment of patient and family to determinehome care needs.  Provides a complete physical assessmentand history of current and previousillness(es)
  • Regularly re-evaluates patient nursing needs.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determinenursing diagnosis.
  • Develops a care plan, which establishesgoals based on nursingdiagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.  Includes the patient and the family inthe planning process.
  • Initiates appropriate preventiveand rehabilitative nursing procedures.  Administers medications and treatments as prescribed bythe physician.  Provides direct patient care as defined in the State Nurse Practice Act.
  • Counselsthe patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate perassessment and plan of care.
  • Identifiesdischarge planning needs as part of the care plan development and implements prior to discharge of the patient.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Communication:

  • Preparesclinical notes and updatesthe primary physician when necessary and at least every 60 days.
  • Communicates with the physician regardingthe patient’s needsand reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
  • Communicates with community health related personsto coordinate the care plan

Job Qualifications:

  • Current Licensure as a Registered Nurse in the State of California and CPR certification.
  • Minimum of 2 years licensed nursing experience in California.
  • Bachelor’sdegree is preferred, with one (1) year of home health care experience desirable. 
  • You must be willing and able to travel extensively throughout assigned regions in California, including working independently in unfamiliar environments.
  • Must possess a valid California Driver’s License and have a reliable, insured vehicle in good working condition, as required by organizational policy.  
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
  • Prolonged or considerable walking or standing.Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.

Physical Requirements:

  • The ability to sit occasionally for 1-3 hours.  
  • The ability to frequently stand for 3-6 hours. 
  • The ability to occasionally drive/ operate foot control for 1-3 hours. 
  • The ability to walk intermittently on level and or unlevel surface for less than 1 hour. 
  • The ability to intermittently bend or climb for less than 1 hour.  
  • The ability to occasionally twist, kneel, crouch and crawl for 1-3 hours. 
  • The ability to lift 50 pounds frequently for 3-6 hours. 
  • The ability to lift greater than 50 pounds occasionally for 1-3 hours.
