Nurse Practitioner/ Physician Assistant, Advance Lung Disease (ALD)/Lung Transplant

The Nurse Practitioner, ALD/Lung Transplant is responsible for providing care in an advanced practice nursing role, under approved protocols and in accordance with the appropriate scope of practice.

Provides ALD, Pre and Post Lung Transplant medical management in an ambulatory clinic setting including general and preventative care, under supervision. Conducts physical examinations and orders diagnostic testing prescribes medications/treatment for ALD/Lung Transplant.

GENERAL DUTIES

Facilitates and maintains a safe environment for patients and others:

Utilizes the nursing process to deliver patient care:

  • Assesses the physical condition of patients by performing physical examinations and obtaining    medical histories. Identifies health and medical risk factors.
  • Formulates the plan of care and prognosis based on patient’s condition.
  • Plans appropriate care, formulates treatment plan, and evaluates patient’s response to that care   under the direction of the Medical Director.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess the patient’s

Clinical  healthcare needs.

  • Initiates and/or recommends medical drug therapies or other forms of related  therapeutic procedures in accordance with approved protocols/policy.

Collaborates with interdisciplinary team members to identify actions to improve patient care:

  • Collaborates with physicians and other members of the multi-disciplinary team.
  • Acts as a liaison and effectively interacts with physicians and staff.

Facilitates continuity of care:

  • Initiates and/or maintains interventions to assure continuity of safe/effective care of patients.
  • Provides counsel to patients, families, and significant others about cardiovascular     

Health,  illness and promote maintenance.

  • Documents consistent with established guidelines.
  • Comply with medical records policies regarding the complete and accurate

documentation  of the care provided.

Leadership:

  • Delegates to other team members to accomplish direct patient care or support activities.

EXPERIENCE QUALIFICATIONS

  • Preferred: Adult Cardiothoracic, cardiology or Pulmonary experience.


EDUCATION QUALIFICATIONS

  • Required: Successful completion of an accredited Nurse Practitioner program.
  • Required: Master's Degree in Nursing-Nurse Practitioner (MSN-NP). (CHNOLA)
  • Preferred: Doctoral Degree in Nursing. (CHNOLA)

Performs assessment of individual health, analyzes data to determine medical and nursing care needed, plans a treatment strategy to meet identified needs, evaluates responses to nursing and medical interventions. Trauma ICU Bedside care in a Trauma ICU in a Level I Trauma Center Working with Dr. Patrick Greiffenstein, TICU director, Dr. Allison Smith, Trauma Medical Director, and all of the TICU attendings Clinical practice will include the full scope of an APP’s practice: bedside clinical care, documentation, procedures, professional billing, etc Integration into the UMCNO Trauma Service as a member of the Trauma Team GENERAL DUTIES Assessment: Assesses the complex acute, critical and chronically ill patients for urgent and emergent conditions. Uses relevant data to evaluate for any physiologic instability and utilizes data to formulate plan of care. Clinical Interventions: Performs therapeutic interventions to stabilize acute and critical health problems. Specific interventions are listed in the Scope of Practice and approved as a part of the credentialing/privileging process. Analyzes the indications, contradictions, risk of complications and cost-benefits of therapeutic interventions. Plan of Care: Manages the plan of care through evaluation, modification and documentation according to the patient's response to therapy. Manages changes in condition with a focus on optimizing the patient outcomes. Incorporates health promotion, health protection and injury prevention measures into the plan of care within the context of the complex acute, critical and chronic illness. Facilitates the patients transition between and within health care settings. Education: Serves as an educational resource to the patient/family, nursing staff and provider team. Acts as a consultant to multidisciplinary health care team members. Professional Development: Participates in scholarly activities including research and education. Participates in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational offerings. Acts as a preceptor and mentor to support the development of nurses, APPs, students and other health care providers. Patient Safety and Quality: Demonstrates a commitment to patient safety by speaking up about safety issues and changing practice to enhance safety. Demonstrates shared accountability for safe practice. Shares errors or potential errors through system reporting mechanisms. Education: • A master’s degree in Acute Care Nurse Practitioner or Physician Assistant Studies.
