Colorado State Veterans Community Living Center at Rifle


The Colorado State Veterans Community Living Center (VCLC) at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including an 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouses, and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. All staff members of Rifle are committed to providing a life of quality and dignity for every resident. Provides basic hands-on patient/resident care in a safe, clean, infection controlled environment by bathing, dressing, grooming, hygiene, skin care, range of motion, positioning, ambulating, transferring, toileting programs, feeding programs, transporting, performing basic assessments (vital signs, blood pressures, weights, etc.), and charting/documenting all of the above.


Primary Job Duties:


Daily Nursing Care

  • Performs basic nursing care: bathing, feeding, dressing, transferring, ambulating, positioning, lifting, range of motion, toileting, skin care, and interacting socially to meet residents' basic needs, and FMP Programs.
  • Assist nursing staff in resident safety, importance of Park and Walk programs, resident transfer and mobility, assistive devices, mobility aids, equipment and gait belts. Perform CPR as needed.

Safety

  • Safety of residents and others by use of correct lifting techniques, monitoring the environment and resident whereabouts/activities, use of safety devices such as lifts, etc.
  • Performs basic resident assessments such as weights, bowel and bladder incontinence, intake and outputs, vital signs, and records the same.
  • Records hall sheet data each shift.
  • Implements proper infection control practices via the use of standard precautions.
  • Equipment checks, equipment maintenance, and placement of equipment, i.e. mattresses on beds, foot cradles, etc.

Communication

  • Communicates with others to ensure proper resident care and work completion.
  • Inform RN's and LPN's of findings and changes in resident condition.
  • Maintains therapeutic milieu through appropriate interpersonal skill use, monitoring of environments and assisting residents to maintain rooms and possessions.
  • Maintains resident care and equipment such as cleaning oxygen concentrators, filling portable oxygen tank, maintain/disinfect tubs and showers, changing lift batteries, etc.
  • Participate in restorative meeting/GAPA meetings.

Transports

  • Transports residents for medical appointments, etc. in state vehicles when the need arises.
  • Attends educational training and staff meetings to maintain current on techniques and policies.
  • Provide adequate inter-shift communication.


Minimum Qualifications

  • A high school diploma or GED ~and~ At least one (1) year of professional experience as a CNA or performing similar duties/assignments of a CNA. 

Education and Work Experience Substitutions:

  • A valid nurse aide certification (CNA) from the Colorado Department of Regulatory Agencies for the high school diploma or GED ~or~ A valid practical nursing license (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact for the nurse aide certification (CNA) can be substituted for the education and work experience

This position requires the following as conditions of employment:

  • Must pass a Colorado Bureau of Investigations and a Federal Bureau of Investigations background check as a condition of employment. Included in the background check is a current and/or previous employer check. Your current and/or previous employers within the past 10 years at a minimum must be listed on your job application (most recent in descending order), even if the job or duties are unrelated to the position for which you are applying. Incomplete employer information (address, phone number, and supervisor) or omission of former employers may result in a disqualification of your application.
  • Must pass TB test.
  • Must be at least 18 years of age
  • Must possess a Colorado Driver's License.
  • Must maintain a current and active CNA License.
  • Must maintain a current CPR Card and perform CPR adequate compressions and ventilations per CPR guidelines.
  • This position is "Essential Service Designation": you will be required to report without delay or interruption to provide essential or emergency services to ensure health, safety, and welfare.
  • This position has shift work: day, evening and night shifts to include holiday and weekends as needed.
  • This position requires on-call work on weekends.
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.

There are typically 300 deliveries per month. The nurse-to-patient ratio is 1:6 - 8, and a CNA in the nursery. Mom/baby nurses also act as “admit” RNs for all infants transferred to the Mom & Baby unit. Nurses take care of many babies whose status does not require NICU care, but who are not yet ready to go home. Mom/Baby nurses may periodically care for a number of GYN patients. Job Summary Under close supervision, assists professional nursing and care providers in providing care to all patients with respect for their uniqueness. Collaborates with the RN and healthcare team. Assists with basic patient care and activities of daily living. Maintains unit specific documentation as required. Orders, maintains and utilizes supplies for the assigned unit. Essential Functions : Provides a safe environment for all patents. Assuring proper patient positions and patient care throughout the patient’s stay. (10%) Assists the RN/LPN in carrying out the established plan of care for assigned patients.
