The Certified Nursing Assistant is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction, and supervision of the registered nurse (RN).

Responsibilities

•Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.•Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.

Qualifications

•CNA license certification prior to hiring.•Must have a current CPR and fingerprint clearance card.•Minimum of six (6) months CNA/LNA experience preferred.•Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience) •LTC/SNF/Rehab preferred.

Responsibilities will vary based on individual needs and may include various services, from companionship and meal preparation to light housekeeping, transportation, medication reminders, and even personal care such as bathing and dressing! Minimum Qualifications: A minimum of 1,000 hours of experience working with geriatric or chronically ill individuals is preferred (approximately 6 months full-time or 12 months part-time). Exceptions are made for recently licensed CNAs or HHAs, or for those providing companionship and housekeeping services only. A CNA or HHA license is a plus, but not mandatory. Current TB testing is required. You must be physically able to perform the duties of this position, which may include transferring clients using proper safety techniques and equipment. If you will be transporting clients, reliable transportation with a valid driver's license and car insurance is a must. Excellent communication skills are essential for effectively interacting with clients, care managers, and other team members. A mature, caring, punctual, and reliable demeanor is key to building client trust and fostering a collaborative environment.
