Responsibilities:

Provide direct nursing care to residents, including administering medications and treatments as prescribed
Monitor residents' health conditions and collaborate with the Charge Nurse and interdisciplinary team to ensure proper care
Deliver care for residents with specialized needs, including neurological conditions, dementia, dialysis, IV therapy, infectious disease, and wound care
Assist in implementing individualized Care Plans and support residents' overall well-being
Maintain proper documentation, follow facility policies, and ensure compliance with state regulations
Foster positive interactions with residents, families, and healthcare staff
Perform additional duties as assigned to support resident and facility needs

Qualifications:

Active LPN license in Pennsylvania, in good standing
Graduate of an accredited nursing program
Strong clinical, communication, and organizational skills
