Responsibilities:

Provide nursing care to residents with various needs, including therapeutic, neurological, dementia, dialysis, IV therapy, infectious disease, and wound care
Administer prescribed medications and treatments, ensuring accurate documentation and resident safety
Monitor residents’ conditions and collaborate with the Charge Nurse and interdisciplinary team to address care needs
Support residents' physical, emotional, and psychosocial well-being
Assist in implementing and following individualized Care Plans
Maintain compliance with facility policies, state regulations, and resident care standards
Perform additional duties as assigned to meet resident and facility needs

Qualifications:

Graduate of an accredited nursing program
Active LPN license in Pennsylvania, in good standing
