Responsibilities:

Provide hands-on care, including bathing, dressing, hygiene, and grooming, in accordance with individualized Care Plans
Assist residents with mobility and ensure their comfort and safety
Deliver care for residents with various needs, including neurological conditions, dementia, dialysis, IV therapy, infectious diseases, and wound care
Work collaboratively with the Charge Nurse and interdisciplinary team to meet residents' needs
Foster positive interactions with residents, families, and staff
Follow facility policies and procedures while performing assigned duties

Qualifications:

Active CNA certification from a State-approved program (must provide documentation upon application)
High school diploma or GED preferred
Strong interpersonal and communication skills
