Qualifications:

  • Certified Nursing Assistant (CNA) license (required)
  • 12 months of caregiving experience (required) (Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice)
  • Current CPR certification (preferred)
  • Hospice experience (preferred)
  • Excellent customer service skills
  • Strong communication (written, verbal, and active listening)
  • Problem-solving skills

  • Ability to work in an electronic medical record system

Responsibilities:

  • Review patient care plans
  • Assist with lifting and transferring patients
  • Assist with range of motion, positioning, exercise, and ambulation
  • Assist with dining and nutrition
  • Maintain electronic medical records
  • Work under the direction of a Case Manager RN

Cervical, thoracic and lumber spine surgical patients are also cared for on this unit. The Medical Surgical population includes respiratory patients with pneumonia, COPD, and Asthma. A range of Medical Surgical patients including appendectomies, gall bladder removal, blood clots and/or kidney stones also make up the patient population. A member of the interdependent team led by a designated RN, is accountable to perform patient care duties and activities for a group of patients. Performs such care as: vital sign checks, bladder scanning, blood sugar testing, am bulates with patients, transfers, helps patient eat, showers and bathes patients, provides incontinence care, repositioning of patients, & helps with oral cares. Documentation of cares provided and r eports patient care data and progress of outcomes to designated RN.
