Job Responsibilities

Receiving orders and care plans from a virtual care team to assist patients during their acute phase of illness
Coordinating with virtual nurses and hospitalists to identify patient needs and any interventions they may require in the hospital at home setting
Providing transition of care support and participating in home visits
Obtaining information from patients about specific symptoms and medical questions
Measuring and monitoring blood pressure, vital signs, and the overall condition of patients
Providing timely and accurate reporting to the virtual care team
Assisting patients with their activities of daily living (ADLs) such as personal hygiene, mobility, and toileting
Offering a compassionate ear to patients and providing much-valued companionship
Maintaining patient engagement by establishing rapport, demonstrating empathy, and building a trusting relationship

Qualifications and Skills

Active, unencumbered Ohio STNA license
Experience in an acute care, in-patient, hospital setting
