Job Responsibilities

Receiving orders and care plans from a virtual care team and preparing daily patient assessments for patients during the acute phases of their illnesses
Collaborating with virtual nurses and hospitalists to identify needs and interventions for hospital at home patients and providing ongoing support for advanced care planning
Participating in home visits and transition of care management
Administering nursing interventions, such as vascular access care, infusion, and wound care
Providing coaching and assistance to patients regarding treatment decisions as well as other health education support
Making evidence-based clinical decisions that align with medical necessity guidelines, practice standards, industry standards, best practices, and contractual requirements
Complying with all requirements related to scope of practice, facility policies, and legal directives
Leading clinical operations and medical management across the entire continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating)

Qualifications and Skills

Active Ohio state Registered Nurse (RN) license

Ability to travel to multiple patients' homes during your shift and to work independently
