Job Responsibilities

Collaborating with virtual nurses and hospitalists to identify needs and interventions for hospital at home patients and provide ongoing support for advanced care planning
Implementing orders and care plans from a virtual care team
Creating daily patient assessments for patients during the acute phases of their illnesses
Participating in at-home visits and managing transition of care
Administering vascular access care, infusion, wound care, and other nursing interventions
Leading clinical operations across the entire continuum of care — assessing, planning, implementing, coordinating, monitoring, and evaluating
Coaching and assisting patients with treatment decisions and providing other health education support

Qualifications and Skills

Active Ohio state Registered Nurse (RN) license
Minimum 1 year of experience in an acute care, in-patient, hospital setting
Ability to place a peripheral line with experience maintaining and accessing midlines and PICCs
Current CPR for Healthcare Providers Certification from the American Heart Association
Ability to travel to multiple patients' homes during your shift and to work independently
Strong clinical and assessment skills
Ability to lift a minimum of 50 lbs
