Job Responsibilities
- Collaborating with virtual nurses and hospitalists to identify needs and interventions for hospital at home patients and provide ongoing support for advanced care planning
- Implementing orders and care plans from a virtual care team
- Creating daily patient assessments for patients during the acute phases of their illnesses
- Participating in at-home visits and managing transition of care
- Administering vascular access care, infusion, wound care, and other nursing interventions
- Leading clinical operations across the entire continuum of care — assessing, planning, implementing, coordinating, monitoring, and evaluating
- Coaching and assisting patients with treatment decisions and providing other health education support
- Making clinical decisions that are evidence-based and align with medical necessity guidelines, practice standards, industry standards, best practices, and contractual requirements
- Following all directives and limitations related to scope of practice, facility policies, and legal requirements
- Receiving and documenting information from patients about specific symptoms and questions
- Fostering patient engagement by demonstrating empathy and taking steps to develop a trusting relationship
Qualifications and Skills
- Active Ohio state Registered Nurse (RN) license
- Minimum 1 year of experience in an acute care, in-patient, hospital setting
- Ability to place a peripheral line with experience maintaining and accessing midlines and PICCs
- Current CPR for Healthcare Providers Certification from the American Heart Association
- Ability to travel to multiple patients' homes during your shift and to work independently
- Strong clinical and assessment skills
- Ability to lift a minimum of 50 lbs
- Hepatitis B and MMR records, and current influenza vaccination
