Job Responsibilities
- Using orders and care plans from a virtual care team to support patients during acute phases of illness
- Working with hospitalists and virtual nurses to determine patient needs and interventions that may be required in the hospital at home setting
- Providing transition of care support and assisting in patient home visits
- Taking and monitoring patients' blood pressure and other vital signs
- Obtaining health histories from patients and information about their conditions and symptoms
- Reporting accurate medical information to the virtual care team in a timely fashion
- Helping with the activities of daily living (ADLs) of patients, such as daily hygiene, eating, mobility, and toileting
- Providing companionship to patients and relaying their concerns to the care team
- Supporting a trusting patient-provider relationship by engaging with patients and demonstrating empathy
Qualifications and Skills
- Active, unencumbered Ohio STNA license
- Experience working in an acute care, in-patient, hospital setting
- Current American Heart Association CPR for Healthcare Providers Certification
- Ability to lift a minimum of 50 lbs
- Hepatitis B and MMR records
- Current influenza vaccination
