Job Responsibilities

Using orders and care plans from a virtual care team to support patients during acute phases of illness
Working with hospitalists and virtual nurses to determine patient needs and interventions that may be required in the hospital at home setting
Providing transition of care support and assisting in patient home visits
Taking and monitoring patients' blood pressure and other vital signs
Obtaining health histories from patients and information about their conditions and symptoms
Reporting accurate medical information to the virtual care team in a timely fashion
Helping with the activities of daily living (ADLs) of patients, such as daily hygiene, eating, mobility, and toileting
