Job Responsibilities

Receiving orders and care plans from a virtual care team for patients during their acute phase of illness
Working with virtual nurses and hospitalists to identify interventions and other needs for patients in the hospital at home setting
Providing comprehensive daily patient assessments and ongoing support for advanced care planning
Administering vascular access care, wound care, infusion, and other nursing interventions
Providing patients with health education and coaching regarding treatment decisions
Making evidence-based clinical decisions that align with scope of practice, medical necessity guidelines, practice standards, industry standards, best practices, contractual requirements, and legal directives
Leading clinical operations and providing medical management across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating)
Participating in home visits and transition of care management for patients
Obtaining, documenting, and reporting information about specific symptoms and questions
Promoting patient engagement through empathetic care that develops rapport and builds trusting relationships

Qualifications and Skills

Active New Jersey state Registered Nurse (RN) license
At least 1 year of experience in an acute care, in-patient, hospital setting
Ability to place a peripheral line with experience maintaining and accessing PICCs and midlines
CPR for Healthcare Providers Certification from the American Heart Association
Strong clinical and assessment skills
