About the position:The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home & determine the patient’s eligibility for home care services & develop the plan of care to be followed. Qualifications:

The hospital provides 24-hour emergency services. We serve children, adolescents, and adults for psychiatric and behavioral health. Duties include: assess mental health needs, develop nursing diagnoses, identify outcomes, plan and evaluate nursing care; administer and/or monitor the effects and side effects of medications and treatments ordered by a physician; to ensure safety and provide milieu management and crisis intervention strategies. Job Duties/Responsibilities Provide leadership and direction to licensed vocational nurses, psychiatric technicians, and counselors. Direct milieu management. Provide assessment of patients’ psychiatric and medical status. Contribute to treatment plan as a member of interdisciplinary team.
