Job Description The Stroke Navigator assists the Stroke Medical Director and Stroke Program Director to improve the quality/delivery of ischemic and hemorrhagic stroke care throughout the continuum of care. He/she works in collaboration with the program directors, data analysts, department leaders, staff and patients to develop and support the stroke program’s performance improvement initiatives. The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each patient including support for interdisciplinary and cross facility collaboration. In addition, he/she identifies opportunities to streamline care practice, using LEAN methodology, through development of evidence-based guidelines and protocols. Job Description The Registered Nurse who works in the school setting is required to work with a high level of autonomy, as they are the only medical professional on campus. Job Description The Clinical Access Nurse assists with the preliminary step of the utilization management process and serves as the access point for admissions. Is responsible for obtaining orders on all patients for appropriate status and level of care using InterQual criteria. Obtains precertification and insurance verification on transfers from other facilities and works in conjunction with patient care services for proper bed utilization and assignment. Responsibilities Quality and Compliance Determines medical necessity and level of care guidelines using InterQual criteria for admissions. Has an understanding of specific criteria required for admission, concurrent stay and an understanding of level of care and criteria subsets within each level of care. Ensures the compliance of EMTALA/COBRA regulations as it applies to transfers from outside facilities. Promotes the quality and efficiency of his/her own performance through participation in staff educational programs, approved continuing education courses, and specialized skill training programs. Integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum to include patient and family education. Relies on education and training to complete responsibilities within scope of licensure as registered nurse. Responsibilities Clinical Practice and Care Management Performs psychosocial/physiologic assessments that integrates changing data. Formulates a plan of care based on trends of similar patients, including patient education and discharge planning. Tailors and prioritizes caring practice to individual needs, including cultural/ ethical/ spiritual needs. Participates in the planning of routine transitional health care needs. (i.e. treatment options patient placement options, end of life options) Responds to changes in patient's needs and provides holistic care using independent clinical judgment based on knowledge drawn from education and experience. Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content. Integrates education during the delivery of patient care. Collaborates with patients/ families to identify realistic desired outcomes. Ensures efficiency in the delivery of care determines immediate priorities and offers solution for problems. Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict. Identifies variances from expected outcomes. Evaluates patient outcomes and makes revisions in the plan of care. Documents all patient care activities as per documentation standards. Assists the healthcare team with the documentation process. Communication and Collaboration Consistently communicates/collaborates with the health care team members, patients and families to maximize patient outcomes. Appropriately delegates patient care activities to other health care team members and participates in making and revising staff assignments to achieve maximum productivity. Maintains knowledge regarding unit initiatives and incorporates the outcomes of the team/ committees work into practice. Participates on unit teams, work groups and/ or committees. Anticipates needs and voluntarily assists others. Quality Demonstrates knowledge of unit-based quality improvement initiatives. Participates in quality improvement initiatives by utilizing standards, guidelines, and pathways for care delivery. Incorporates evidence-based practice and research findings into nursing practice to enhance outcomes. Professional Development Acts as a clinical resource and serves as a mentor to health care team members; displays leadership behaviors. Identifies own learning needs based on self-evaluation and the progress made from the previous year's goals. Accountable for all documentation and completion of all required continuing education and competencies relevant to area of practice as defined by clinical management. Ensures improvements in practice settings by assuming responsibility for self-development in life-long learning. Qualifications Graduate of accredited school of nursing Proficient in the English language, verbal and written communication skills, computer skills Current Louisiana State license as RN and BLS certification
