Consults with and provides education to other IDT members on a prn basis for clinical issues relating to palliation Actively participates in the IDT. Communicates effectively, timely, and appropriately at all times. Informs physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care. Completes face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period afterwards. Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned. Uses standard precautions at all times, and per agency policy. Adheres to agency policies regarding protected health information. Keeps up with and maintains education/knowledge in pharmacology and palliative medicine Provides on-call services to patients/families as assigned. Overview NaphCare is hiring a Full Time Days Nurse Practitioner (NP) to join our team of medical professionals in Tucson, Arizona, where we manage all aspects of healthcare for the individuals who are currently incarcerated in the Pima County Detention Center . As a Correctional Nurse Practitioner (NP) , you will work alongside correctional officers and other medical professionals to provide care to the individuals in residence. Working within a controlled environment, where security is top priority and the days are never dull, you will have various duties to keep you engaged including assessing, diagnosing and treating patients with a range of medical conditions and chronic illnesses, prescribe medications, respond to emergencies, etc. This is a unique position where you can make a difference to this diverse population of patients with complex medical needs. Responsibilities Job Duties / Responsibilities The Nurse Practitioner plays a critical role in our continued mission to provide quality care that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and are not limited to: Conducting physical examinations, developing treatment plans and monitoring patients Managing acute and chronic conditions such as asthma, hypertension, and diabetes.
