In collaboration with other members of the healthcare team, the Case Manager is accountable for coordinating and facilitating the management of care for an assigned group of patients. The goal is to ensure a well-coordinated, individualized care experience through outcome-oriented care within a fiscally responsible framework. This includes early assessment of discharge needs, resource availability, and communication among healthcare team members and families, spanning pre- and post-hospital care. I. Major Responsibilities: 1. Assessment: Evaluate appropriateness of inpatient admission and assess patient needs to identify high-risk individuals and initiate referrals. 2. Multidisciplinary Practice Guidelines: Participate in the development and monitoring of interdisciplinary guidelines and variance tracking. 3. Collaborative Practice: Work with physicians and healthcare providers to enhance quality of care and utilization management. 4. Discharge Planning: Facilitate timely discharge through early intervention and coordination of services. 5. Data Management: Monitor compliance, collect utilization data, and identify cases for quality and utilization review. II. Position Qualifications: License/Certification/Education: Required: 1. BSN 2. Current Massachusetts RN license Preferred: 1. CCM certification Experience/Skills: Required: 1. 3–5 years of clinical experience with sound judgment and knowledge of current medical care and support systems 2. Job Summary : The Physician Assistant provides comprehensive primary care to patients within an outpatient environment. They will coordinate the care of patients under the supervision of a Board-Certified Physician. In addition, the Physician Assistant acts as a member of the clinical team and works collaboratively with other departments in providing patient care. Essential Functions: Delivers direct primary care and preventative services to assigned children, adolescent, and adult patients. Develops therapeutic plans of care for prevention, management, and maintenance of good health for patients. Provides health education and counseling patients. Provides care for Acute and Chronic Illness and Urgent Care services, which includes assessment, diagnosis, treatment, and monitoring of illness. Works closely with multidisciplinary teams and instructs and assists other team members in management of patient care. Follows established practice standards and current evidence-based clinical practices and guidelines. Provides care related to prenatal/postnatal services and care for uncomplicated pregnancies, in conjunction with physician partners, as delineated and approved for in request for clinical privileges. Provides family planning counseling and services. Consults with physician partner and refers patients with more complex/dynamic problems to subspecialty care, as appropriate. Maintains and updates Electronic Health Records (EHR) in a systematic format and completes required documentation within the specified amount of time expected by the Clinical Director. Ensures that patient records are kept confidential consistent with Health Center policies and procedures, Joint Commission, and HIPAA standards. Monitors HIPAA compliance within the work environment. Attends trainings, weekly Provider and Team meetings, and collaborates on practice, administrative, and quality improvement projects, as assigned by Clinical Director Provides clinical supervision and mentoring to Physician Assistant, Medical, and Physician Assistant students by mutual agreement with the Clinical Director. May work a fluctuating schedule, including days, evenings, weekend shifts, and holidays according to schedule approved by Clinical Director. Performs other duties as assigned. Education and Experience: Master’s Degree as a Physician Assistant. Summary The Internal Medicine Physician is a member of the clinical practice team and works collaboratively with other staff members to provide high quality patient care according to the PCMH model. The Internal Physician will lead the care for their own diverse panel of patients. This Provider will manage chronic diseases and treat acute conditions in patients across the adult life span. Internal Medicine Physicians at Kennedy Community Health will have the opportunity to work in partnership with the numerous other disciplines offered at the health center including mental and behavioral health, pharmacy, Medication Assisted Treatment (MAT) program, Reproductive Health, Dental and Optometry. Essential Duties & Responsibilities Provides medical care to patients with acute medical problems, chronic illnesses, or requesting preventive health services. Examines patients, formulates diagnostic plans, and orders diagnostic testing. Interprets examination findings and test results and implements treatment plans. Provides medical care and performs procedures within the scope of reproductive/pregnancy health including IUD and Nexplanon insertions and removals, cervical examinations, and when appropriate providing prenatal care to pregnant patients. Determines the need for consultation and assists in medical care and treatment provided by other specialists. Participates in other care procedures according to training and demonstrated ability. Prescribes and assesses effectiveness of pharmaceuticals, other medications and treatment regimens as appropriate for patient’s medical conditions. Meets the Health Center’s annual productivity goal. Maintains and updates Electronic Health Records (EHR) in a systematic format and completes required documentation within 48 hours. Participate in after hours and Weekend call on a rotating basis according to the schedule assigned by the Medical Director. Required Qualifications Graduate of an accredited medical school (M.D., or D.O.). MA Board of Registration to practice medicine. Completion of an Internal Medicine Residency program. Board Certification in Internal Medicine (ABMS) within 1 year of Residency completion.
