Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Assisting clients with medications reminders which are ordinarily self-administered. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. Radiation Oncology Full Time 86954BR Job Summary Under the supervision of the Administrative Nurse (AN) or Charge Nurse, the Clinical Nurse II (CNII) is responsible for providing skilled and comprehensive nursing care to Radiation Oncology patients with a wide variety of oncological and/or disease related problems seen for consultation, treatment, and follow-up. Through application of the nursing process, the CNII provides direct care and implements plans for continuity and coordination of patient care with other members of the patient’s health care team. In collaboration with the AN, the CNII provides appropriate guidance and supervision for ancillary nursing personnel and participates in orientation of new medical and/or technical personnel. Responsibilities include compliance with Medical Center and departmental Policies and Procedures, Environmental Safety, House and Telephone Standards, compliance with all regulatory agency requirements. The position may involve travel between all UCSF Radiation Oncology sites. The hours are generally daytime but may vary with start times from 6:00 AM to 11:00 AM and end times may vary from 3:00 PM to 8:00 PM. The position may occasionally involve a few hours of on-call at the end of each day. This role will be primarily at our Mission Bay Campus working four 8-hour days M-F in our HDR Brachytherapy Procedure Suite, but may include working in the ambulatory clinic as well at any of our UCSF Health Radiation Oncology locations. This is an exciting opportunity to work with a passionate group of physicians who are a pleasure to work with. The Brachytherapy procedure is performed primarily in the Rad Onc department with urgent cases occasionally added to the Adult OR. When the procedure is performed in the department, often there us optimal patient experience due to the decreased overall length of the procedure day. The CNII will perform pre-procedure screening/assessments; assist the Anesthesiologist and the Radiation Oncologist during the procedure; recover the patient post anesthesia; prepare the patient for discharge home or admission per instruction/ protocol and follow up with patients after their visit. Ideal candidates will have a procedural, OR, PACU, ICU or ED skill set/background. In addition to the HDR procedure, the nurses support the daily radiation patients by managing their side effects, dressing changes, teaching, explaining, advocating for the patients’ needs, answering questions, navigating them to the appropriate providers, and providing emotional support. Many patients come daily M-F for several weeks, and the nurses have opportunity to build therapeutic relationships with the patients. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
