Oncology Patient Navigator, RN

Requirements:

Education: Bachelor’s or Master’s degree in nursing from an accredited school of nursing preferred.

Experience: Minimum 2 years Oncology experience preferred. Experience in multiple hospital departments preferred.

Licenses/Certification: RN License current in the state of Colorado or compact state. Current BLS certification. ONC Certification at time of hire or within 1 year of hire.

Responsibilities:

As the Oncology Nurse Navigator at Gunnison Valley Health you’ll be responsible to provide individualized assistance to patients, families and caregivers; to guide the patient and the family through the health system from diagnosis, testing, treatment and follow up care. You will utilize your unique skills and abilities by:

Facilitating care within the context of functional status, cultural consideration, health literacy, psychosocial and spiritual needs for patients.
Developing and maintaining an effective liaison role between patient and other medical team members.
Educating the patient, family, and caregivers regarding treatment options, community resources, and self-care management throughout all phases of the process.
Partnering with the GVH Marketing team to conduct ongoing promotion of the healthcare program.

Physical Requirements:

Occasionally - (1-33%) Walking, sitting, crouching/squatting, stooping, stairs, lifting (0-20 lbs), carrying (0-20 lbs), pushing/pulling (20-50 lbs)

Frequently - (34 - 66%) Standing, walking, change position, reaching, reach across midline, handling, pinching

Continuously - (67 -100%) Must be able to see with corrective eyewear, must be able to hear clearly with assistance

Benefits Eligibility:

Medical, dental, vision, healthcare FSA, dependent care FSA, and Lifestyle Spending Account: All active employees working 40 or more hours per pay period in a Full Time or a Part Time position are eligible for benefits on the first of the month after hire.

Life and AD&D insurance: All active employees working 40 or more hours per pay period are eligible for benefits on the first of the month after hire date.

Short term and long term disability: All active employees working 60 or more hours per pay period are eligible for benefits on the first of the month after hire date.

Compensation: $36.51 - $47.00 /hour DOE
