Case Manager - RN:
Position Summary:
Manages each patient hospital stay by working with hospital staff, physicians, and insurers, providing optimal care that is cost effective and timely; Identifies, coordinates and communicates discharge/transfer needs; Identifies correct status and provides advisement to hospital team; Completes clinical reviews and ensures authorization for services.
Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Typically reports to a head of a department/unit. Must be a registered nurse with 2-4 years of clinical experience. Familiar with standard concepts, practices, and procedures within a particular field.

Licensure/Certification/Education (Required):
- Current/Active Florida Registered Nurse License (RN) OR Approved Multi-State License
- Education 3: Technical School/2 Years College/Associates Degree
