AS A HOSPICE LICENSED PRACTICAL NURSE (LPN): you will administer nursing care to terminally ill Hospice patients, as needed, in the patient's place of residence in accordance with physician orders and plan of care and in accordance with agency Hospice policies and procedures.

Qualifications- Have a current PA Practical Nurse License
- Have previous Home Health or Hospice experience (preferred)
- Have a valid PA driver's license and reliable transportation
- Have 2 years of LPN acute care or long-term care experience
- Have completed a Hospice orientation/training program prior to providing care
- Have excellent time management skills
- Have strong verbal and written communication skills
- Have ability to exercise initiative and independent judgement
- Have ability to work independently and within a multidisciplinary team
- Have ability to pass PA Child Abuse History Clearance
- Have ability to pass Criminal Background Check
- Have ability to meet physical requirements of position
- Have acceptance of philosophy and goals of the Hospice Program
