Summary:
The RN Acute Care Case Manager evaluates the patient’s health status and the patient/family/significant other’s care capabilities to facilitate the discharge planning process. The case manager develops a multidisciplinary, culturally appropriate, age-specific discharge plan. They also complete clinical reviews utilizing Internal criteria for the appropriate level of care and insurance coverage.
Qualifications:
- Bachelor's degree in a healthcare-related field or equivalent experience.
- ASN/BSN preferred but not required
- 3 – 5 years of case management-related health care experience. A minimum of 7 years of experience is required without a Bachelor's degree
- Care Coordination
- Excellent written and verbal communication skills. Basic Microsoft Office.
- Able to achieve the following competencies within 90 days:
- Abuse reporting
- Guardianship
- Discharge planning/Case Coordination
- Referral to community resources
- Utilization review
