Description:
Under the general supervision of the Director of Care Coordination, this position acts as the clinical liaison between patients and health care providers to ensure continuity and coordination of care for patients at moderate to high risk for rehospitalization who are transitioning from hospital to home or another care facility/provider.
Bacherlor's of Science in Nursing, Registered Nurse licensed in the State of Pennsylvania, Case Management certification from an accredited organization preferred. Three (3) years of case management experience, including emphasis on discharge planning.
