Summary: Reporting directly to the Clinical Manager of Cardiac Associates, this individual will coordinate and supervise the overall function of the Heart Valve Clinic at Concord Hospital. This will require working collaboratively with administration, physicians, nurses, and ancillary personnel. The Clinic Coordinator facilitates coordination of care and communication with each patient, and partners with physicians in the development of treatment plans. He/She will communicate with the multidisciplinary health care team and ensure regulatory requirements are met. Provides excellent leadership, organizational, communication, and interpersonal skills and works in a self-directed environment. The Clinic Coordinator will provide community and physician education on the Heart Valve Clinic. This position will also collect and aggregate clinical data, and maintain required registries. Education: Bachelor of Science in Nursing (BSN) required, MSN preferred. Experience: Effective interpersonal, organizational and motivational skills required with ability to function as a team player, and proficiency in written and verbal communication. General knowledge of medical office operations, including scheduling, registration, electronic medical records, patient flow, billing, coding, charge capture, referrals, authorizations, payor guidelines, compliance, credentials, privileges, regulatory agencies, and DNV standards. Minimum three years Cardiac nursing experience required. At least one year in a surgical sub specialty, cardiology or acute care strongly preferred. Experience working with Microsoft Office. Certification, Registration & Licensure: Certification required: BLS, Licensure required: Licensed to practice Nursing in the State of New Hampshire. Responsibilities Clinical oversight and management of the Heart Valve Program and Clinic and Protected Percutaneous Coronary Intervention Program and Clinics with strong emphasis on operations, implementation, and development. Coordinates the continuum of heart valve disorders and complex coronary artery disease patient throughput. Provides outreach education regarding the Heart Valve Protected Percutaneous Coronary Intervention Clinics to community, referring offices, hospital, and office staff. Manages Regulatory data and Fiscal Resources for the Heart Valve and Protected Percutaneous Coronary Intervention Program. Oversees and manages compliance and TAVR registry, including all data reporting, and joint decision making documentation. Participates actively in quality improvement efforts. Works independently and within a team on special, nonrecurring and ongoing projects. Coordinates multiple aspects of projects, events, and other complex activities.
