Glendale Memorial Hospital Medical Center is actively searching for an exceptional Stroke Nurse to provide unparalleled support and expertise to our stroke patients and program.  In this dynamic role, you won't just be a nurse; you'll be a patient's personal health assistant, providing crucial physical assessments, education, guidance, and continuous support throughout their stroke diagnosis and recovery. You'll be the reliable point of contact, ensuring every patient benefits from seamlessly coordinated care.

You'll be instrumental in elevating our stroke program's quality, leading continuous improvement efforts, boosting patient satisfaction, and engaging the community with vital outreach. Working hand-in-hand with our medical team, marketing, and strategy, you'll also contribute to exciting program development and long-term planning, helping us innovate and grow.

  • Conducts daily rounding on all assigned neuroscience program patients, with multiple daily rounds in critical care units.
  • Conducts daily physical assessment on all assigned neuroscience program patients, as appropriate.
  • Assists with immediate detection of any neurological or other medical status change and provides immediate intervention to patients in distress within scope of practice.
  • Documents assessment in medical record and writes appropriate orders based on assessment, within scope of practice and in collaboration with supervising physician. Performs patient history and  physical as requested by MD.
  • Determines eligibility of stroke patients for enrollment in the program
  • Assesses patients' clinical and educational needs, patient medications and therapies. Provides education regarding disease, sign/symptoms, risk factor modification, medication management and diet.

  • Conducts post acute stroke discharge phone calls and follow-up on any coordination of care needs.

  • Communicates and coordinates all necessary information and data related to all projects with all necessary entities

  • Assists with stroke data collection and provides report to the Stroke Program Coordinator and Medical Director.

  • Serves as an expert resources and educator for staff, patients and management regarding chronic disease management

  • Employees may be required to perform other duties as assigned.

Job Requirements
  • Minimum 2 years acute care experience as an RN in ICU or ER units required
  • Current and acitve Registered Nurse: CA (RN:CA)
  • AHA Basic Life Support - CPR (BLS-CPR)
  • AHA Advanced Cardiac Life Support (ACLS)
  • Critical care, cath lab, neurology experience preferred
  • Experience in Patient Care Coordination preferred

Job Summary and Responsibilities As our ICU Nurse at St Mary Medical Center, you’ll monitor patients who are critically ill and unstable following extensive injury, surgery, or life-threatening disease.
