The Supervisor position reports to the Manager, Case Management and is responsible for the direct supervision of the assigned Case Managers, Coordinators, and Case Management Associates. Supervision includes the daily coordination and management of all assigned Case Management staff to ensure excellence in customer service. The Supervisor develops and implements an operational plan that includes staffing resources, policy and procedures, orientation and education, and quality improvement activities under the direction of the Manager and Director of Care Management. The position assures timely provision of coordinated patient care that includes all case management functions of care coordination, regulatory management, discharge planning, and interventions through application of case management standards of care. The Supervisor maintains expertise in case management and ensures that services are aligned with the vision and goals of the department. The Supervisor ensures the development plan of individual and professional group case management staff and ensures that services are aligned with the vision and goals of the department.

Required Qualifications

Bachelor's Degree in Nursing or
Upon hire: Washington Registered Nurse License
3 years Discharge planning and case management in an acute care setting.
3 years Recent experience working with diverse patient populations including various age groups with physical or developmental disabilities, emotional, behavioral, and mental disorders.
Experience working with patients who struggle with substance abuse and knowledge of the medication assisted treatment programs (Methadone/Suboxone).

Preferred Qualifications

Master's Degree in Nursing or Community Health
Coursework/Training: Crucial Accountability Training
Coursework/Training: Emotional Intelligence Training

We have two rooms fully plumbed to handle dialysis patients. Computers are also located in each patient room to facilitate bedside documentation and medication verification. We utilize Epic as our electronic health record. The management structure of the unit consists of an experienced Clinical Manager and a Clinical Coordinator who work together to oversee the unit. The Charge Nurses provide additional clinical support for the staff and help to maintain the flow of the unit. The staff is composed of RNs and Care Assistants, with support services provided by Respiratory Care, 24-hour Pharmacists, PT/OT, IV Therapy RNs, Dieticians and RN Care Managers. RN assignments are determined by patient census and acuity, and our normal RN ratios are 1:5, with CNA support.
