About the Position:

Community Care is seeking a dedicated RN/LPN to provide private duty, one-on-one care for a 19-year-old male patient in their Perryopolis home. As part of our trusted team, you'll deliver personalized care while enjoying the support of a family-run agency that has proudly served local communities for more than three decades.

Available Shifts:

  • Tuesday (Overnight): 10:00 PM – 7:00 AM

  • Thursday: 8:00 AM – 5:00 PM

  • Friday: 7:00 AM – 6:00 PM

Pick up one shift or build a consistent weekly schedule.

Responsibilities Include:

  • G-Tube care and feedings

  • Medication administration

  • Assistance with activities of daily living (ADLs)

  • Routine monitoring and documentation

  • Providing safe, respectful, and patient-centered care in a home environment
