Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (NP/PA-C) for Family Medicine and Pediatrics at Winchester Square on Gender Road in Canal Winchester. The APP, in collaboration with the Physicians, coordinates the healthcare of primary care patients at this location. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress. About the area and practice: Canal Winchester is located fifteen miles southeast of Columbus. It has a small-town ambiance with the convenience of shopping, restaurants, parks, festivals, and historical museums and a twenty-minute drive to Columbus. This COPC practice has established a reputation as a primary care group that serves the community. They provide the best primary care for all ages with a focus on health education, disease prevention and management to keep their patients well. It is a large outpatient practice consisting of (4) family physicians and (3) pediatricians. There are APPs on staff and several medical assistants and patient service reps to support the providers and patients. This APP opening is for the Family Medicine side of the practice. The practice is open Mon-Friday at the following times; Mon and Tues 8-6pm, Wed and Thurs, 7-4 pm, Fri. 8-4 pm. Your schedule can adjust when needed. Key Responsibilities: Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages. Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries. Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD.
