About the Role:

As a Licensed Practical Nurse (LPN) in a hospital-based Skilled Nursing Facility (SNF), you will play a crucial role in providing high-quality patient care during the day shift. Your primary responsibility will be to assist in the daily living activities of patients, ensuring their comfort and well-being while adhering to established care plans. You will collaborate closely with registered nurses and other healthcare professionals to monitor patient progress and report any changes in their condition. This position requires a compassionate approach to care, as you will be interacting with patients and their families, providing education and support. Ultimately, your efforts will contribute to the overall health outcomes and quality of life for the patients in our facility.

Minimum Qualifications:

Current and valid LPN license in the state of Ohio.
Completion of an accredited practical nursing program.
Basic Life Support (BLS) certification.

Preferred Qualifications:

Experience in a skilled nursing facility or hospital setting.
Knowledge of electronic health record (EHR) systems.
Certification in geriatric nursing or related specialty.

Responsibilities:

Administer medications and treatments as prescribed by the healthcare team.
Monitor and document patients' vital signs and health status.
Assist patients with daily living activities, including bathing, dressing, and mobility.
Communicate effectively with patients, families, and healthcare team members to ensure coordinated care.
Participate in care planning and contribute to the evaluation of patient progress.

The required skills for this position include strong clinical assessment abilities, which will be utilized daily to monitor patient conditions and respond to their needs effectively. Excellent communication skills are essential, as you will be interacting with patients, families, and the healthcare team to provide updates and education. Time management skills will help you prioritize tasks and ensure that all patients receive timely care throughout your shift. Additionally, familiarity with EHR systems will enhance your ability to document patient information accurately and efficiently. Preferred skills, such as specialized certifications, will further enhance your capability to provide tailored care to our diverse patient population.
