Pay Range: $40.00 to $44.00/hour

Under the direction of the RCFE Healthcare Director, the Assisted Living lead LVN is responsible for the comprehensive nursing care of all the RCFE residents.
May be responsible for supervision of CNA personnel who assist in providing care to these residents in accordance with current applicable federal, state and local standards and regulations. Assures that the highest degree of quality resident care is maintained at all times. Performs all assigned duties in a professional manner, in accordance with established policies and procedures, nursing procedures, the nurse practice Act and other safety rules and regulations. The Lead LVN may be assigned additional monitoring and administrative duties to assist the RCFE Healthcare Director in the supervision and leadership of the RCFE Healthcare Department.
Requires independent decision making, problem solving skills and the ability to function in stressful situations. Must be able to perform work activities alone, and be able to direct, control, or plan an activity.

Essential Functions
- Receives and follows schedule/instructions from the RCFE Healthcare Director.
- Responsible for doing the 24 hours Assisted Living Report per protocol related to assigned residents.
- Assist residents with medications and treatments (to intact skin surfaces). Evaluates the effectiveness of these medications and treatments by applying education, experience and nursing knowledge.
- On a daily basis be solely responsible and accountable for accurate information on the Medication Administration Record (M.A.R.) for each resident per protocol related to assigned residents.
- Maintain an awareness of resident’s health. Informs both orally and in writing to the clinical leadership and appropriate staff of any incidents or changes in resident condition occurring during the shift. Assures that changes are reflected in the resident’s chart.
- Maintains all records and reports requested by RCFE Healthcare Director, the Clinical Department, or Physician.
- Assist in residents’ self-administration of medication following MHC policies and procedures. Secures discontinued medications following MHC policies and procedures. Orders medications as necessary following MHC policies and procedures.
- Reads reports, reads and implements orders according to MHC policies and procedures.
- Makes multiple decisions based on information input from resident
- Evaluates resident requiring medical/nursing assistance and follows-up as appropriate. Contacts the physician or activates 911 as situation requires following MHC policies and procedures for MedStat and Medical Emergencies as well as any additional appropriate MCH policies and procedures.

- This position requires regular attendance and punctuality as directed by the Time and Attendance policy for MHC.
- Maintains confidentiality of appropriate resident care information to assure their rights are protected.
- Promotes a safe environment which includes fire and accident prevention, maintenance of sanitary conditions, and assurance that staff knows roles and responsibilities in the event of fire or disaster. Reports all incidents/accidents, unsafe and hazardous conditions/equipment immediately.
- Performs testing of residents and/or staff as needed.
- Assists with tasks assigned by the RCFE Healthcare Director including but not limited to, Medication Cart audits, MAR reviews, Physician Order reviews, assistance with scheduling, and staff supervision.

Educational Requirements/Licenses/Certificate
- Possession of a valid license or certification to practice in the State of California as an LVN. Current CPR Certification.
- Must have at least 2-5 years of full-time experience providing direct care to the aged/chronically ill. Two to three years RCFE experience desirable.
- Must have excellent communication skills and interpersonal skills.
- Intermediate knowledge and skill with computers and word processing and spreadsheet software.
