In a perfect world, we would hire a Licensed Practical Nurse who was capable of working with our patients on a 1-1 basis. The ideal candidate will have strong people, communication and administrative skills and will enjoy working in a fast-paced, exciting community. The right candidate will have experience working in a clinical setting with older adults who are suffering from varying degrees of dementia as well as physical disability and cognitive impairment.

Responsibilities:

Work with patients and families to assess their needs and document care plans.
Communicate and interact with patients, families, and other health professionals.
Provide assistance to patients who are performing activities of daily living.
Assist with basic hygiene tasks and assess patient's physical needs.
Prepare medications under doctor's orders.
Provide assistance to patients using equipment such as walkers, wheelchairs, and beds.

Job Type: Full-time, part-time or per diem

Pay: $33.00 - $42.00 per hour

Expected hours: 16 40 per week
