Job Responsibilities

• Working with nursing staff to administer basic nursing care in a post-acute facility
• Ensuring appropriate documentation of the medical history and care for residents
• Reviewing resident’s vital signs and medical charts
• Assessing the physical and mental health of residents on an ongoing basis
• Verifying the correct medications and dosages for residents
• Changing bandages and providing wound care where needed
• Helping to explain medical and facility protocols to residents and family members
• Ensuring the overall health, comfort, and safety of patients

Qualifications and Skills

• High school diploma (or equivalent)
• Completion of an approved LPN educational program
• Applicable state license to practice as a Licensed Practical Nurse (LPN)
• At least 4 months’ experience in post-acute care or in-patient hospital environments
